Symptoms

In 12.2.6 Item information is not displayed in Item field in Service Request Information Region in Charges HTML form

Steps:

(R) Customer Support Specialist
(N) Agent Dashboard
 - Open Existing SR from Service Request Work Queue or Create New SR
 - In the Service Request Update page, select Actions: Create Charges and Service Logistics
