QA collection Plan results entry fails Post Upgrade to 12.2.6

Problem Description

Have a collection plan - triggered during the Work Orderless completion. It has a collection element (Serial Number) which consists of four action SQL's. In 11.5.10.2 , Results entry is working fine without any error. Post Upgrade to 12.2.6, the Results entry is throwing error as below - APP-QA-16116: The value of the collection element that token 'NUM' corresponds to is null. Your action 'Assign a value' may fail. Do you want to continue.

Process works fine in 11.5.10 and same is not working in 12.2.6.
