Symptoms
Profile Option Service: Default Parent Information not properly applied in HTML
Set Profile Option Service: Default Parent Information to Default from Recovered Item Instance. However in the HTML version this behavior is not being applied. Rather no matter what we set this profile to the Instance information from the Service Request header is populated in the Parent Item fields.
Step To Reproduce

1) Provide the details, including the navigation path, on how to consistently reproduce the issue.
Log in using responsibility Customer Support Specialist
Create a Service Request for a "child" item.
Go to Charges
Select an Order type Service activity
Observe that the Parent item is defaulted. This should not happen.
Add the child item to be returned
The correct Parent item does not populate.
