On : 12.2.5 version, Service Request Form

ACTUAL BEHAVIOR
---------------
In the Service Request form, while using a particular IB instance, the Incident Customer and Incident Address are not populated

EXPECTED BEHAVIOR
-----------------------
Incident Customer and Incident Address should be populated

STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Profile option "Service: Address Filter Options" is set to "Customer Sites,Related Party Sites and Locations".
2. Open the Create Service Request form
3. In the Service Request form header, use an Installed Base instance that is owned by part A but is located at Party B
4. Incident Customer and Incident Address are not populated
