Errored Workflows are not showing status of error

Symptoms
On : 12.1 HRMS RUP10 version, Workflow,Approval,Common Funct
When an employee submits a leave request, or other self-service process, and there is a break in the reporting hierarchy, the workflow would normally have a status of 'error'.
The workflows are not displaying a status of 'error' but are showing as 'active'.
There are no error messages received when the workflow runs in error
Steps to Reproduce:
The issue can be reproduced at will with the following steps:
Submit workflow Self service Generic Approval Process
1. (N)System Administrator->Workflow->Status Monitor
- workflow type: HR
- type internal name: HRSSA
- process name: Self service generic approval process
