Service HTML: Price Not Defaulting - Exception Details in Charges page - Unable To Submit Charges

Symptoms
The following issues (one or all) may exist in Service HTML - Charges:
1. Price not defaulting
2. Error when trying submit Charges
3. Error Page: "You have encountered an unexpected error. Please contact your System Administrator for assistance."
Possible Errors:
There are no charges eligible for submission to Oracle Order Management for this service request.

Could not find and load the custom class oracle.apps.cs.charges.server.CsChargeAMImpl
Steps:
- Apply p27332935_R12.CSZ.C_R12_GENERIC.zip (which included ER 27046940)
(In addition to the ER, that patch was intended to address the issue saving with unrelated Parties, where it cleared the Shipping and Billing information),
- Apply Dev-tested patch p27466842_R12.CS.C_R12_GENERIC.zip
(After selecting Bill To and Ship To for an unrelated Party, the page was not refreshing the correct Ship To Party.)

- Query SR
- Navigate to Charges region
- Enter Charges lines
=> Price not defaulted.
- Try to submit Charges
=> Error occurs.
- Try to fix the default issue by selecting SAC value, adding another line, etc.
- Try to submit Charges again
=> Error occurs.
* Note:
For the Price not defaulting:
When adding a second line to the Service Request, the Business Process defaults to a value.
- Chose the Service Activity (i.e. Ship Exchanged)
- Entered item:
=> The price did not default from the [expected] Price List.
- Saved the record
- Then selected the same Service Activity (for that same line)
=> The price defaulted in for both lines (1 and 2)
