Error 'java.lang.NullPointerException' 'initializeCustAccountSiteRec' When Creating Address And Associating To Account

Symptoms

When attempting to creating address and associating to account in HTML contact center,
the following exception error occurs.

STEPS
-----------------------
The issue can be reproduced at will with the following steps:

1) Navigate to (R) Customer Support > Customer Management > HTML Contact Center
2) Query a customer record using account number.
3) Select "Create Address" from Actions Menu on right top side.
4) Capture details of the address, address usage, tick-Yes the option to "Associate Address With Account", select account record, select account site usage.
5) Click the button "Save and Close"
