OAF Pages Are Not Launching In Forms But Are Coming Up In Main Menu (Doc ID 2395119.1)

On: 12.1.3 version, Upgrade related issues
The OAF pages are not launching from forms,
but are coming up from the main menu:

ORA-01403: no data found
FRM-40735:WHEN-LIST-ACTIVATED trigger raised unhandled exception ORA-00600

The issue can be reproduced at will with the following steps:

1. Log into E business Suite as any user.
2. Navigate to a form.
3. Try to open OAF pages via forms.

No change to system was reported when issue began to occur
