Custom Asset Additions ADI Template Error

Symptoms
When attempting to create a new Asset Integrator ADI template for asset additions the following error occurs.
ERROR

The value $PARAM$.CATEGORY_FLEXNUM for OA_FLEX_NUM in the flexfield definition is invalid or empty for the interface column identified by Integrator code: 140:FA_MASS_ADDITIONS, Interface Code 140:FA_MASS_ADD_INTERFACE and sequence number 80 null.
STEPS

The issue can be reproduced at will with the following steps:
From Oracle Asset Responsibility
(M) Mass additions>Additions integrator
1.Select book
2.Click create
3.Click open to create document
