In 12.2.6, Cannot create Task using Customer Support Specialist - Agent Dashboard

Attempting to create a task using Agent Dashboard

Steps To Reproduce
================

1. Go to Agent Dashboard>>navigate to Resolution and Wrap up>>select task type from list of values >> Click Go and Apply>>Error page comes up

The error indicates
oracle.apps.fnd.framework.OAException: java.sql.SQLException: Initialization failed
