Employee Contact Missing Details (Email and Address and Phone No) On Create Service Requests Page (Doc ID 2417196.1)

Symptoms

On : 12.1.3 version, Service Request Form

ACTUAL BEHAVIOR
---------------
In Create Service Requests > Contacts/Addresses tab, only Last Name/First Name and Number are displayed.

EXPECTED BEHAVIOR
-----------------------
On 12.0.6 and previously, are a lot more detail on this drop down (Email Address/Address etc).
Need to see more details about employee on "list of value"

STEPS
-----------------------
The issue can be reproduced at will with the following steps:
Steps to verify the bug is fixed:
(R) Service
(Nav) Service requests > Create Service requests
- Search any SR
- click on Contact / Address tab
In the contact type select as Employee

Last Name / First Name List of value displays Three columns : Last Name,First Name and Number
