Symptoms
On upgrade or change of OS (OS upgrade or clone)
On running sqlplus, getting segmentation fault even though all of the required packages from install note are applied. This applied to both EBS 12.2 and EBS 12.1 typically observed with RHEL and LDAP users.
Issue can be verified by performing:

Change in Operating System (OS), either for a new clone or OS upgrade.
