Service HTML: Covered Customer Name Missing From Agent Dashboard (Doc ID 2431388.1)

Symptoms

ACTUAL BEHAVIOR
---------------
In Agent Dashboard Create Service Request or Update Service Request page, there is a Covered Site field. But there is no Covered Customer field.

EXPECTED BEHAVIOR
-----------------------
There should be a Covered Customer field that displays the customer associated to the Covered Site address

STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Go to Agent Dashboard and either create a new SR or update an existing one
2. In the Create/Update Service Request page, on the header section, there is a Covered Site field.
   But there is no Covered Customer field that shows to which customer the Covered Site belongs to.
