After system maintenance (patching, migration, etc) when the applications tier is restarted, users report the following intermittent login errors:

      "You have encountered and unexpected error. Please contact your System Administrator for assistance.

java.lang.NullPointerException at EDU.oswego.cs.dl.util.concurrent.ConcurrentHashMap.put(ConcurrentHashMap.java:507)at oracle.fnd.util.CHMWrapper.put(CHMWrapper.java:332)at oracle.fnd.util.AppsConcurrentHashMap.put(AppsConcurrentHashMap.java:240)at oracle.apps.fnd.framework.webui.JRAD2AKMapper.getRootMElement(JRAD2AKMapper.java:647)"
