Quality Workbench Will Not Open Or Fails With Error When Trying To Add/Update Data (Doc ID 2451562.1)

Symptoms

When attempting to Open a plan using the Quality Workbench, the Workbench will not open.  Or, it fails with the following error

ERROR
Plan won't open

Exception Details.
oracle.apps.fnd.framework.OAException: oracle.jbo.NoDefException: JBO-25058: Definition InfoFlag of type Attribute not found in ResultHgridVO1
at oracle.apps.fnd.framework.webui.OAPageErrorHandler.prepareException(OAPageErrorHandler.java:1275)
at oracle.apps.fnd.framework.webui.OAPageErrorHandler.processErrors(OAPageErrorHandler.java:1461)

STEPS

The issue can be reproduced at will with the following steps:

1. Apply a Quality Patch or upgrade from 11.5.10.2
2.  Responsibility = Quality
3.  Navigation = Workbench-> Collection Results
4. Query Collection Plan->Select the collection plan (Click on the collection plan)
