Symptoms

Problem Statement
When attempting to submit Requisition for approval, the following error occurs.

Error
PL/SQL procedure: PO_REQAPPROVAL_INIT1.SET_STARTUP_VALUES
Error: ORA-06508: PL/SQL: could not find program unit being called

Steps to Reproduce

  1. Create a Requisition
  2. Submit the Requisition for approval
  3. Receive below title notification:
    "Approval workflow database error occurs while approving Purchase Requisition..."
