Tax Jurisdiction field missing and Tax Jurisdiction description is missing from transactions form

Tax jurisdiction field and tax jurisdiction fields should be present

STEPS

The issue can be reproduced at will with the following steps:
1. Query transaction and hit tax details button
2. Notice that at bottom of field tax jurisdiction field and tax jurisdiction description is missing

The issue has the following business impact:
Due to this issue, users no longer see description of jurisdiction.
