Goal

Plan Name = Spouse Medical Surcharge
Standard Rate Calculation Method = Flat Amount
Flat Amount = 200.00
Program Activity Reference Period = Biweekly
Enrollment/Rate Frequency = Per Pay Period with Frequency Rules (could also occur using Per Pay Period and Estimated Per Pay Period)

Why does the Confirmation Statement page in Self-Service > Benefits display a monthly amount of $433 instead of the per pay period amount of $200 for this plan?

All plans are defined with a Flat Amount Standard Rate.
