Concurrent Requests Hang Intermittently On Windows

Symptoms
On : 12.1.3 version, Request Processing
Concurrent requests hang intermittently. Same concurrent requests complete successfully sometimes. The affected requests are mostly for FSG program.
Issue happens in both Seeded (FSG report) and Custom requests.
The following error is seen in the ICM log:

Could not submit job to kill request session <Session ID>:
Could not submit job to kill concurrent process <Process ID>: Oracle error 100: ORA-01403: no data found has been detected in FND_CONC_RAC_UTILS.SUBMIT_MANAGER_KILL_SESSION.
