TeleService: Notes not Appearing in the 'Log And Notes' Window for Some Service Requests (Doc ID 2472683.1)

Symptoms

On : 12.1.3 version, Service Request Form

ACTUAL BEHAVIOR
---------------
After creating a Note for a Service Request (SR), the same is not visible in the 'Log and Notes' window.


EXPECTED BEHAVIOR
-----------------------
Any note created should be visible on "Log And Notes" page.

 

STEPS
-----------------------

1. (R) Service
2. (N) Service Requests > Find Service Request

3. Search for a Service Request with this issue 
4. Navigate to Workbench tab and and check that a note exists in the Notes section
5. Click on the "Log and Notes" button

=> Just a black screen is displayed
