Meeting Error "ORA-01017: invalid username/password; logon denied" When Running ADOP (Doc ID 2484305.1)

Symptoms

On : 12.2.6 version, Online Patching - OP

When attempting to run ADOP, the following error occurs.

Enter the WLSADMIN  user password:

Validating credentials.

Initializing.
  Run Edition context : /<DIR>/NODENAME.xml
  Patch edition context: <DIR>/NODENAME.xml
  Patch file system free space: 380.15 GB

Validating system setup.
  Node registry is valid.
  [WARNING] ETCC: The following required database fixes have not been applied to node NODENAME:
  25919622
  Refer to My Oracle Support Knowledge Document 1594274.1 for instructions.

Checking for existing adop sessions.
  Continuing with existing session [Session ID: 13].
   Session Id : 13
   Prepare phase status : COMPLETED
   Apply phase status : ATLEAST ONE PATCH IS ALREADY APPLIED
   Cutover phase status : NOT COMPLETED
   Abort phase status : NOT COMPLETED
   Session status : FAILED
