On : 12.1.3 version, Spares Logistics
When attempting to create an order for a part requirement, the following errors occur:
ERROR

1. Value for cust_account_id - party_site_id must be unique.
2. Value for orig_system_reference must be unique"
STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Create part requirement.
2. Try to create order.
