Trading Partner Setup Automatically Being Deleted From The Form But Showing Via The Backend Tables

Symptoms
On : 12.1.3 version, Transaction Entry
When attempting to setup Trading Partner the system is automatically
deleting it from the application, but shows via the backend and the
following error occurs.
OBSERVATION
Observed some issue where trading partner setup for tp code- XXXXX-YYY
is not showing in oracle form/screen but we could see in back end table.
select * from ECE_TP_HEADERS where 1=1
and TP_DESCRIPTION = 'ZZZZZ YYY'
When trying to create the same setup it is giving us the error APP-FND-01206
ERROR
APP-FND-1206 This record already exists. You entered a duplicate value or sequence of values that must be unique for every record.
STEPS
1. Responsibility: Release Management
2. Setup TP_DESCRIPTION = 'ZZZZZ YYY'
3. Trading Partner is Deleted
4. Try to setup TP again > Observe: APP-FND-01206
