After Upgrading the Database to 12c, the PDF Associated with E-Records is not Generated or not Accessible (Doc ID 2487783.1)

On : 12.1.3 version, Workflow Issues

Upgraded the database over weekend and since then this is failing. This is production where this is not working.
Note: have one env which is also upgraded and that works.

ERES attachment not generating as expected.
The E-Record in the Formula Approval does not contain the Attached Document.
In the E-Record the attached file does not exist although the message "Review the attached e-record document, eRecord_3063752.pdf " does appear.

Note: Verified profile Applications JSP Agent. The setting is "http://hostname:port" (without the quotes, and using the actual hostname and port number) and this has not resolved the issue.

E-record attachments to work correctly.

The issue can be reproduced at will with the following steps:
1. Responsibility: Formulator
2. Formulas
3. Change status, generate e-record from status change.
4. View e-record, click on attachment and no document appears.
