Symptoms

ACTUAL BEHAVIOR
---------------
When the Extract Process is run, DTP values are duplicated only for one dependent.

EXPECTED BEHAVIOR
-----------------------
DTP values must not be duplicated for this dependent.

STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Navigate to Processes and Reports > Submit Processes and Reports.
2. Select Single Request .
3. Submit Extract Process with the appropriate values.
4. Review the Extract Process output.
DTP values are duplicated for one dependent.
