TeleService: Service Request Notes Type And Description Do Not Show On Form (Doc ID 2528131.1)

Symptoms

Once you have created a Note for a Service Request (SR) and you re-query the SR, the existing Notes do not show the Note Type or Description fields once you click on them.
You can only see the data in the grid. The details do not populate when you click on each Note.

Steps:

(R) Service
(N) Service Requests > Create Service Requests

 - Query up an existing SR that has any Notes created
 - Navigate to the Workbench tab.
 - Click on that existing note.

 => Details of the note do not populate in the fields below.

Workaround:

Click the 'Refresh' button.

Notes:

Applying Patch 23210956:R12.CAC.C - LINE BREAKS AND CARRIAGE RETURNS NOT DISPLAYED PROPERLY IN PRINT FRIENDLY is recommended, but it is not the definite solution for this issue.
