Symptoms

Unable to add new instance relationship type code.

When attempting to add a new instance relationship type, the following error displays in the 'Instance Relation Types' form

Not able to add record here.

STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Install Base Administrator -> Setups - > Instance Relation Types

2. Attempt to add a new row
