Provide steps for an upgrade to 12.2.1.3.0 from previous 11g BI Publisher versions.

Recently many upgrade recommendations have been made regarding 12.2.1.3.0, and users are looking for the best way to migrate from older versions.

For example an upgrade case might include the following versions:

1. BI publisher 11.1.1.5.0 on Redhat linux 5.7 - machine A
2. BI publisher 11.1.1.9.0 on Redhat linux 5.7 - machine B

The customer might look to upgrade them to new machines C & D with either Redhat linux 6 or 7, which would also involve an O/S upgrade.
