When attempting to create a New Item Request (NIR), it results in unexpected error page.

The issue can be reproduced at will with the following steps:
1. Development Manager >Item Catalog > Item Simple Search
2. Search Item: > Go
3. Click on Item
4. Actions > Create New Item Request
5. Create New
6. Continue
