ACTUAL BEHAVIOR
-------------------------
When creating Customer Verification Question (Dynamic), unable to apply/save the record. As soon as fields are populated, they 'disappear'.

EXPECTED BEHAVIOR
----------------------------
Expect the fields to populate as they are entered.


STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Populate the customer verification question (dynamic) fields
2. Once the fields are populated the data disappears.
3. Unable to apply/save the record.

BUSINESS IMPACT
-----------------------
The issue has the following business impact:
Due to this issue, users cannot apply and save the record as the data in the fields disappear.
