Service Request Tasks Tab: FRM-41092: No Records In Block CREATE_TASK (Doc ID 2557576.1)

Symptoms

When attempting to click on the Tasks tab of the Service Request form an error occurs.
This error started after applying one of the latest Service cumulative patches (i.e. 29653416:R12.CS.B 12.1.3:BUG FIXES FOR CS:APR'19)

Error:

FRM-41092: No records in block CREATE_TASK
FRM-40735: WHEN-TAB-PAGE-CHANGED trigger raised unhandled exception ORA-06502

Steps:

(R) Service
(N) Service Requests > Find / Create Service Requests
- Query Service Request
- Click on the Tasks tab

The error above is displayed.

What is Working:

- SR can be created
- Tasks can be created

Users may also encounter the error reported in <Note 2453998.1>:
APP-JTF-210604 ... A grid must be specified using BLOCK.ITEM_NAME in UPPERCASE
