User Login Fails With "A connection to the server could not be established: null" Error. (Doc ID 2576856.1)

Symptoms

OTM users are getting the following error when trying to login.

A connection to the server could not be established: null

Replication steps
---------------------
01. Log in two different browser (BR1, BR2)
02. On BR1 :
02.1 Change the user password
02.2 Log out
2.3 Log in with the new password
03. On BR2 yet opened and logged with old password
03.1 Click on a menu ( the system give an exception message)
03.2 Repeat the step 3.1 5 times
04 Open a new page and try to login.
