Unable to launch Account Hierarchy Manager.
Navigate to Setup > Accounts > Manager and attempt to open the Account Hierarchy Manager, a new browser is initiated, a message is returned to indicate that a new applet has started, then completed, but no dialog box returned to ask whether user wants read-only or read-write access.
