Cannot Add Values to Lookup IT_PROVINCE Error States they Already Exist (Doc ID 2619157.1)

Problem Statement:

We are trying to add two values (FL - Florence) and (TU - Turin) to the IT_PROVINCE lookup we get the following error:

APP-FND-01206: This record already exists. You entered a
duplicate value or sequence of values that must be unique
for every record.

Steps to Reproduce:
The issue can be reproduced at will with the following steps:

1. XX HRMS Manager

2. Other Definitions

3. Query IT_PROVINCE
