Symptoms
On : 12.1.3 version, SUPPORT
When attempting to create a CHS Disposition Plan (child plan) element from an existing CHS Defect Plan (parent plan) element
the following error occurs.
ERROR

"FRM-40735: WHEN-NEW-FORM-INSTANCE trigger raised unhandled exception ORA-06508".
STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1.Create open defect on CHS Defect Plan with the action set to In_Process. Save
Navigate to Results > Entry > Update Quality Results > CHS DEFECT PLAN
2. Locate the open defect without disposition
3.Update ‘Action’ element to correct disposition action
4.Select appropriate child plan from drop down menu on bottom of window. Click Enter
5. Child Plan will open with new disposition element. Enter any relevant information.
Save
6. Exit from Child Plan and save Parent Plan. Get error
