Requisition Is Automatically Approved After Two Timeout Notifications Without Any Update On Them

Purchase Requisition is submitted for approval, but is pre-approved as it is within limit of preparer.
The Approver does not action the email notification (does not Approve or Reject).
Timeout occurs and 2 notification reminders are sent - still no action taken against them.
Requisition is not returned to the requestor with a status showing the unapproved action, as Incomplete for ex - it appears with an Approved status (and shows as such in the Action History), although final Approver did not actually approve it.
So the Requisition is automatically approved after 2 timeout notifications and instead it is expected that it should be returned to the Preparer indicating that no approver has been found.
The same applies to Purchase Orders.
