Symptoms

When using "Delete Constraints"

ACTUAL BEHAVIOR
---------------
"Delete Constraints" form not allowing to create new records

EXPECTED BEHAVIOR
-----------------------
"Delete Constraints" form should allow user to create new records.

STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Inventory -> Setup -> Items -> Delete Constraints
2. Form Opens - all fields are grayed out.
