Symptoms

Purchase Orders (POs) are getting stuck in 'In Process' status with the error as shown below

This can happen after an upgrade from application release version from R12.1.3 to R12.2.9.

Error Message: ORA-04063: package body "APPS.PO_REQAPPROVAL_ACTION" has errors ORA-06508: PL/SQL: could not find program unit being called: "APPS.PO_REQAPPROVAL_ACTION"
Error Stack: Wf_Engine_Util.Function_Call(PO_REQAPPROVAL_ACTION.OPEN_DOC_STATE, POAPPRV, 7426896-808726, 311806, RUN)

 

STEPS  TO  REPRODUCE

The issue can be reproduced at will with the following steps:
Step 1: Create a PO --> Submit for Approval --> PO Gets stuck in 'In Process' status and PO Approval workflow completes in error.
