Symptoms
When attempting to enter annual leave for one employee
the following error occurs.

FRM-40735: ON-INSERT trigger raised unhandled exception ORA-01422 when creating absence request using Absence Form
STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. (N) HRMS Manager responsibility -> People -> Absence form
2. Enter details -> Save
3. Error is raised
