500 Internal Server Error on REST Web Service

Symptoms
There is an ISG web service call that is working fine for several days. But after several executions, it starts giving 500 Internal Server Error.
Once the '500 Internal Server Error' starts (after thousands of proper executions) the web service will continue giving error even after clearing Cache and Bouncing Middle Tier.
Only database bounce resolves this issue.
The oafm_server1.log shows the following error:
<oracle.tip.adapter.apps.AppsContextManager> SQLException: ORA-20001: Oracle error -1422: ORA-01422: exact fetch returns more than requested number of rows has been detected in FND_LOG.INIT_TRANSACTION.
