Unable To Save Work Order In 12.2.5 Due to Error: Scheduling Job and creating material requirements (Doc ID 2748396.1)

Symptoms

On WIP - Work in Process, 12.2.5 version, Work Order Definition

After creating a Work Order in Work in Process, while saving the WO, we are getting a message below:

ERROR

Scheduling Job and creating material requirements...

STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. (R) Work in Process
2. (N) WIP > Discrete > Discrete Jobs (form)
3. Click New(B) and enter the details
4. Save.

ERROR IN THE FND DEBUG LOG:

Session <session id>:   info: ORA-01001: invalid cursor
ORA-06512: at ""APPS.FND_ATTACHED_DOCUMENTS2_PKG"", line 1007
ORA-00001: unique constraint (APPLSYS.FND_ATTACHED_DOCUMENTS_U1) violated in Package wip_bomRoutingUtil_pvt Procedure explodeRouting
