AME : PO Action History Only Shows Submit Although Purchase Order Is Approved

Symptoms
ACTUAL BEHAVIOR
Action history does not show the APPROVE action when approving the PO document using AME even though the PO is approved.
EXPECTED BEHAVIOR
Action history should reflect the APPROVE action by the approver
STEPS
The issue can be reproduced at will with the following steps:
1. Submit PO for approval
2. PO is approved but action history only has the SUBMIT action.
