New User Cannot Approve Requisition Due to Build Default Approval List Error ORA-065612

Symptoms
A new user has been setup and cannot approve requisitions and the documents are failing approval
and are getting stuck in an in-process status. Other users can approve requisitions
without any problems. The documents are failing in the approval workflow with the following
exception in the Workflow Activities as follows:
Oracle Workflow Directory Service Synchronization is claimed to have not been implemented.
