2020 ACA State of California XML Rejected for Multiple Missing Elements after Patch (Doc ID 2781567.1)

This Note covers multiple patches released to resolve multiple missing elements in the State of CA ACA XML file.
The error itself is fairly generic - but the missing elements are typically stated at the end of the error message.

Here's the first one received in the chain:

XML Schema Validation Failed – not well formed or missing required elements. Detail: javax.xml.transform.TransformerException: org.xml.sax.SAXParseException; lineNumber: 2; columnNumber: 1; cvc-complex-type.2.4.d: Invalid content was found starting with element 'hci:MonthlyCoverage'. No child element is expected at this point.

Steps to Reproduce
The issue can be reproduced at will with the following steps:
1. <Patch 32575269:R12.BEN.C> (if 12.2) or <Patch 32575269:R12.BEN.B> (if 12.1) have already been applied.
2. Go to responsibility US Super HRMS Manager.
3. Navigate to Processes and Reports > Submit Processes and Reports > Single Request.
4. Select Request Name "Benefits ACA State Transmission Generation Process" and submit with the following parameters:
-Year = 2020
-GRE = <gre_id>
-Filing Indicator = P
-Validate = Commit
-State = California
5. Transmit file to FTB.
