iStore Login and Password Assistance page is Blank (Doc ID 2790703.1)

Symptoms



ACTUAL BEHAVIOR
---------------

Login and Password assistance is not working in iStore flow.
After user inputs user name and email then click 'email my password' button
  a blank page is seen
 

EXPECTED BEHAVIOR
-----------------------
Expect to see a confirmation page acknowledging the action


STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Access Customer UI
2. Click Login
3. Click "Forgot your Username/Password information?" link
4. Enter username and email information, and click "email my password" button
