Action History is Not Appearing For Second Level Approvers in PO Approval Notifications (Doc ID 2797247.1)

Symptoms

On : 12.2.6 version, Approvals - Req, PO

ACTUAL BEHAVIOR:
Action history is not appearing in the notification sent to the second level approver for PO document approval.

i.e.:

Action history section of the notification for the first approver shows information as follows:

 

However, for the second level approver, the action history does not show any information and the exception "No Results Found" under the "Num" column as seen below:

 

EXPECTED BEHAVIOR:
Action history should show in the notification for all approver levels

Steps:
The issue can be reproduced at will with the following steps:
 1. Submit PO document for approval
 2. First approval acknowledge approval notification and progress the document approval
 3. Second approver receives the notification to perform the approval action, that notification is missing the action history information
