Symptoms
Forgot password throws below error after entering password incorrectly 3 times.

Message not found. Application: FND, Message Name: UMX_LOGIN_HELP_UNEXPECTED_ERROR.
java.sql.SQLException: ORA-01086: savepoint 'WF_SAVEPOINT' never established in this session or is invalid ORA-20002: 3101: Status row for item 'UMXLHELP/88274', activity ID '266621' does not exist. STEPS

The issue can be reproduced at will with the following steps:
1) Enter your password incorrectly 3 times on the login page
2) Click the forgot your password link
3) Enter your user name in the User Name field and click 'Forgot Password'
4) Above error occur is thrown.
