Symptoms

While upgrading from R12.1.3 to R12.2.10, the following error occurs while applying CUP11 and merged upgrade driver patch

ERROR

R12.2 Upgrade driver fails as apps.FND_USER_PKG is invalid

12.2 Upgrade driver has a new issue now qppolicy.sql on worker 1 is failing now

ATTENTION: All workers either have failed or are waiting:

FAILED: file qppolicy.sql on worker 1.

ATTENTION: Please fix the above failed worker(s) so the manager can continue.
