Incorrect Action History for the notifications with multiple approvals (Doc ID 2824668.1)

Symptoms

Default action history section it is showing duplicate records when the approval  process has more than one level.

Package wf_notification contains procedure GetComments2. This procedure builds action history for the notification. There are 2 cursors which are showing duplicated data if one workflow item contains more than one notification (more than one notification id per Workflow Item).
