OPM Lot Cost Items Setup: Unable to Assign Items to Lot Cost Type

User has defined a new Lot Cost Type, then navigates to the Lot Cost Items form to assign items. After entering some items and saving the records it was noticed that no data was actually saved. If the User reopens the form, there are no items shown. The following error also occurs:

FRM-40735: WHEN-NEW-FORM-INSTANCE trigger raised unhandled exception ORA-01403.

STEPS

The issue can be reproduced at will with the following steps:

1. Responsibility: OPM Financials
2. Navigate: Lot Costs > Setup > Lot Cost Items
3. Enter: Lot Cost Type, enter items, then Save.
4. After reopening the Lot Cost Items form no items are shown for the Lot Cost Type.
